Vaccination Card Collection

As we continue to navigate another unique year due to the COVID-19 health and safety guidelines, we are focused on two primary goals:

  • To ensure safe in-person learning

  • To reduce the number of quarantined students whenever possible

Per the health guidance, when an asymptomatic, vaccinated student is identified as a close contact, there is no need for them to quarantine. Therefore, one of the ways we can further our goals is by offering families the opportunity to upload a student’s vaccination card to Skyward.

Key Points

  • Students ages 5 and up are eligible for the Pfizer COVID-19 vaccine

  • Uploading the vaccination card is optional

  • Individual student vaccination information will remain confidential

  • Vaccine cards should only be shared via Skyward

  • The purpose of the upload is to reduce quarantines and keep students in school

To Upload Your Card

  • Login to Skyward

  • Click the "Online Forms" button

  • Find the "Covid 19 Vaccincation Card" form and click "Fill out form"

  • Click "Choose File" to upload your vaccincation card

  • Upload your card

  • Click "Complete Step 1 and Move to Step 2"

  • Enter the information into the fields

  • Click "Complete Step 2 and Move to Step 3"

  • Click "Submit COVID-19 Vaccination Card"

Puberty 1 Program- 5th Grade

As part of our fifth grade curriculum, students will participate in a Puberty I program provided in partnership with Candor Health Education (formerly known as Robert Crown Health Center). Below is information on the student program as well as an invitation for parents to preview the presentation through January 31. The FAQ for parents on the Puberty I program can be read here.

Parent Presentation: To view the pre-recorded parent presentation click here.  Use the password found below.  The pre-recorded parent presentation will be available through January 31.

Password: POV1012

Student Program: The student program will be held during school hours in February/March, on a date and time determined by each school. Your child’s principal will be sending out more information including dates/times and a student permission slip.

Student Booklet: Below is electronic access to the booklets that your child will receive as part of the Puberty I program. These booklets provide information on the physical changes that young people experience during puberty. Please note that both classes and booklets include basic information about the anatomy of the opposite sex.

  • Male Program:  Puberty I Male Booklet

  • Female Program:  Puberty I Female Booklet

We know that parents are the most influential role in a child’s life. For ideas on how to use the booklet, general tips for talking with your child and ways to get conversations started, click here. If you would like more information about the center, or the program your child will attend, please visit the Candor Health Education website at If you have any additional questions, please contact Candor Health Education at (630)325-1900.

Cold Weather Reminder

Please send students in winter coats, hats and mittens each day. Our goal is to go outside for recess and a mask break as often as possible. The children need the opportunity to get some fresh air and run around.

We consider both temperature and wind chill when making the decision about mask breaks. Please note that as a general rule, if the “feels like” temperature is at zero degrees or higher, we will most likely be going outside, even if just for a very short time.

CogAT Reports (2nd and 5th Grade Families)

During the week of November 8-12, your 2nd of 5th grade child completed the Cognitive Abilities Test (CogAT). This is an assessment that indicates the level and pattern of cognitive development of a student in comparison to grade and age peers.

The CogAT is comprised of three sections:

  • Verbal: measures a child’s ability to remember and transform sequences of words, to understand them and to make inferences and judgements about them.

  • Quantitative: measures a child’s understanding of relational mathematical concepts and ability to discover patterns, to figure out a rule or principle, and to transfer this information.

  • Non-Verbal: measures a child’s ability to reason using pictures and shapes. In addition, this section appraises a child’s ability to use his/her cognitive resources in new situations.

The individual score report will be sent home on Friday, January 14th, and it includes your child’s standard score, percentile rank, and stanine in each of the three areas as well as a composite score (VQN) for the entire assessment.
