A message from Neysa Guerino, Executive Director
CONGRATULATIONS to all participating practices in the CSMS-IPA / Cigna 2016-2017 collaboration!  Together we have achieved success and a shared savings distribution of more than $600,000 has been made to our engaged practices.    The care delivered to the 36,000+ Cigna members attributed to our network was outstanding.  It met quality standards and demonstrated efficient use of resources. 
We appreciate everyone’s effort that contributed to our Cigna collaboration success and we are anxiously awaiting year end results from our other collaborations.  Our data shows steady growth in achievement of quality metrics and value-based activities.  
As we enter the second half of 2018 we ask you to focus on the completion of preventative care visits and the management of your high risk patients.  We encourage you to use the Annual Wellness Visits and/or Comprehensive Physical Exam to close care gaps and develop care plans with your patients.  Participation in clinical rounds is a great opportunity to review complex patients and identify health plan resources that may be helpful to your patient.  If you are interested in participating in clinical rounds or need guidance on priority activities to achieve success, the CSMS-IPA staff is here to support you so do not hesitate to reach out to us!   
Clinical Rounds:
Goal to improve the health outcomes of your high risk patients

CSMS-IPA and our payer partners are collaborating with our practices to review high risk patients through clinical rounds. In a team huddle approach, we review patient trends based on claims.  The practice is asked to offer specific insight based on the medical record of the patient, laboratory tests, specialists care and screenings.  Together we identify additional resources that are available to the patient from the office, payer, and employer that may be helpful in achieving desired health outcomes of the patient.   
Feedback from practices that have already participated in clinical rounds suggest an increased awareness of its high risk patients.  We ask all practices to participate in rounds annually.  For more information and to schedule a clinical round for your practice rounds please contact your CSMS-IPA resource.  
CSMS-IPA would like to offer an opportunity to you to participate in a free trial subscription of a patient education resource, CareKitsOnline™.  This web based service provides access to over 55 Self-Care Disease Management Guides. The resources may be printed and shared with your patients to assist in the management of chronic conditions.     
Here are some specifics to consider in your review: 
Trial Period:  CSMS-IPA 90-day free trial period is June 1, 2018- August 31, 2018. Upon completion of the trial period you will be automatically enrolled in CareKitsOnline™, unless you notify the vendor of discontinuance by August 31, 2018. 
Service Agreement:  Once the pilot period ends, each interested practice will sign their own service agreement directly with CareKitsOnline™, and will be issued a unique User ID and password.
Fees:  Negotiated cost of the program will be $25.00 per month for any CSMS-IPA practice after the completion of the 90-day trial period or August 31, 2018.  The agreement has a 30-day notice of termination with or without cause after the trial period. 

Please Note:  It is the responsibility of the practice to cancel the agreement if you are no longer interested continuing your subscription.  The vendor will contact you during the pilot to determine if you want to continue. 
Evaluation: Trial practice participants may be asked to complete an evaluation of the program to assess the program benefits.

There is no obligation to participate in this trial.  If you are interested in participating you are required to complete the CareKitOnline™ Services Agreement and return it to Communication Science Inc. 1550 Barclay Blvd, Buffalo Grove, IL 60089 or FAX to 847-850-7506.  It is important to include your practice logo if you would like the material co-branded.

2018 CSMS-IPA/WellCare Medicare Advantage Annual Wellness Visit Initiative
Our newly expanded WellCare collaboration includes the opportunity to earn bonus funds for completion of Annual Wellness Visits (AWVs) and meeting quality metrics.  In an effort to help you achieve the bonus funds we are excited to launch an important initiative designed to improve the AWV rate of completion.  CSMS-IPA is coordinating a mailing, on behalf of interested practices, to all WellCare patients attributed to your practice that have not had an AWV completed this year.  The letter will provide information to the patient on the importance of preventative care and request they contact you to schedule a visit.  WellCare is also offering to make follow-up phone calls to your patients so we can achieve success.  Our network goal is to reach 80% by September 30, 2018.  For more information please contact  Pattie Mastriano at or call our operations office at (203) 225-1291.

CTHealthLink Offers Unique Benefits to CSMS-IPA Members
In May, CSMS-IPA and CTHealthLink entered into a new and exciting initiative aimed at providing members with access to the expansive analytics and tools available in the physician-led, statewide health information exchange (HIE). Designed to help improve clinical outcomes, reduce inefficiencies, and positively impact patient safety, CTHealthLink is delivered in partnership with the Connecticut State Medical Society.
Through CTHealthLink, participating physicians can securely and efficiently access patients’ health records, including accessing patients’ data across disparate healthcare systems. Physicians and patients benefit from the reduction in treatment delays and enhancement of clinical decision making.
The new collaboration with CTHealthLink provides you with reliable, tested, HIE technology, including secure messaging with participating providers.
Participation in CTHealthLink may help you:
  • Enhance patient encounters;
  • Improve care coordination;
  • Increase workflow and productivity; and
  • Eliminate duplicate labs, tests, and x-rays.
The new CSMS-IPA collaboration with CTHealthLink aligns with the organization’s mission to create partnerships between business and medicine that support a value-based system of care, and the opportunity to assist physicians in achieving the highest quality of care, the greatest patient satisfaction, with the most efficient use of innovative resources and technology.

Medicare is in the process of mailing out new Medicare cards to all participants.  
Below please find helpful information for you and your patients.

All Medicare Administrative Contractor (MAC) secure portal Medicare Beneficiary Identifier (MBI) look-up tools are ready for use. If you do not already have access, sign up for your MAC’s portal to use the tool. Once CMS mails the new Medicare card with the MBI to your patient, you can look up MBIs for your Medicare patients when they do not or cannot give them. If the tool indicates the card has not been mailed for your Medicare patient who lives in a geographic location where the card mailing is finished, tell your patient to call 1-800-Medicare (1-800-633-4227).
To ensure people with Medicare continue to get health care services, continue to use the Health Insurance Claim Number (HICN) through December 31, 2019 or until your patient brings in their new card with the new number.
Check this website as the mailings progress. Continue to direct people with Medicare to for information about the mailings and to sign up to get email about the status of card mailings in their state. 
CMS is committed to mailing new cards to all people with Medicare by April 2019.
Information on the transition to the new Medicare Beneficiary identifier: Print and give them the “Still Waiting for Your New Card?” handout (in English or Spanish).
Or tell them to call 1-800-Medicare (1-800-633-4227). There might be something that needs to be corrected, such as updating their mailing address.
