Empowering Women to Prioritize Herself

Designed for a woman to be an active member of her health care team, Healthy Women Houston works individually with each pregnant woman to ensure her medical, emotional and social needs are being met.

The Healthy Woman Houston Community Care Coordinator (C3) is a community-based peer that is an advocate for a woman's care. The Healthy Women Houston medical team – consisting of physicians and nurses in community clinics and hospitals – knows each woman, instilling a culture of respect that puts a woman first in her care, building trust and supporting women in their own communities.

Healthy Women Houston and our collaborative partners have come together to mend the fractured approach to women’s health, getting women into care sooner, leading with a culture of respect, and improving the quality of care.

Community Care Coordinator Spotlight: Tianca


Tianca is a Community Care Coordinator from The Council on Recovery and serves pregnant and postpartum women in the Third Ward and Sunnyside communities. She is a chef, mom, birth doula, and a birth activist. She received her doula certification from the International Doula Institute in 2019. Her inspiration to enter into the world of birth work, maternal health, and advocacy came from her own struggles with navigating the complexities that exist in the system, while pregnant with her son. Being a lifelong resident of South Park, her dream is to one day create her own birthing center within the community she calls home.

Agency Spotlight: Memorial Hermann Health System

Healthy Women Houston is a collaboration among seven agencies committed to improving maternal health in Harris County: Avenue 360 Health & Wellness, Community Health Choice, The Council on Recovery, Harris County Domestic Violence Coordinating Council, HOPE Clinic, Interfaith Ministries for Greater Houston, and Memorial Hermann Health System.

Memorial Hermann Health System is redefining health care for the individuals and many diverse populations it serves. With a vision of creating healthier communities, now and for generations to come, the Memorial Hermann Southwest and Memorial Hermann - Texas Medical Center hospitals offer Healthy Women Houston members a patient-centered, multidisciplinary approach that delivers safe, comprehensive, quality care to women of all ages. The labor and delivery service uses evidence-based standards of maternity care and coordinates its hospital services with the outpatient services available at the federally qualified health clinic. Certified registered nurses work with some of the nation’s best doctors to ensure that every woman served has a memorable, positive childbirth experience.
