Rory Jaffe, MD MBA
Executive Director
California Hospital Patient Safety Organization
Rory Jaffe is executive director of the California Hospital Patient Safety Organization, dedicated to eliminating preventable patient harm, and serving more than 300 facilities in the western United States.
“The 1999 Institute of Medicine report 'To Err is Human' pointed out that eliminating preventable patient harm requires redesigning processes and equipment to reduce the harm to patients caused by human error, recognizing that human error is inevitable and should be expected even from the best practitioners. The new small-bore connectors will markedly reduce a problem first recognized 50 years ago, very rare but often fatal misconnections leading to wrong-route delivery of drugs, fluids or gases.”
