The State of Colorado is offering gift cards and vouchers while supplies last to anyone who receives a first or second dose of a COVID-19 vaccine at a participating state-operated vaccine site. Moderately and severely immunocompromised people who are recommended to receive an additional vaccine dose are also eligible for a gift card or voucher.
Beginning Monday, August 23, 2021, and while supplies last, vaccine sites may offer either $100 Walmart gift cards (one per person), $20 Chipotle gift cards (two per person), or $50 Colorado Parks and Wildlife vouchers (one per person).
Coloradans can register for a vaccine at a gift card stop in advance and check back regularly for any additional gift card clinics at cocomebackcash.com. Walk-ups are also accommodated.
Also this week, The Filipino American Community of Colorado (FACC) pop-clinic on Aug. 24 will offer a free meal from Chuey Fu’s Food Truck to the first 100 vaccinated.

CDHS has a requirement to aid in the data collection necessary for HB21-1100: Electronic Filing of Government Documents. The requested deadline from OIT for this inventory is August 31st in order to meet their reporting requirements to the Governor’s Office. More information about this requirement will be coming to teams from the Executive Management Team.
Ultimately, the future goal is to ensure that forms are electronically available to citizens to the greatest extent possible. As we experienced this past year, our ability to serve constituents relied heavily on technology and electronic formats, and we are working to ensure this access is available as much as possible moving forward.
Our first step toward this goal is to compile an inventory of all forms, whether electronic or paper, used across CDHS for services provided to our clients and customers.
What’s needed from you:
- All forms used by CDHS for obtaining information from citizens must be listed on this spreadsheet.
- Guidance on how to fill out the spreadsheet is on the first tab
There will be open office hours for questions on the following dates:
- Tuesday, Aug. 24 from 2-3 p.m.
- Thursday, Aug. 26 from 10-10:50 a.m.

It’s that time of year! Mid-Year Evaluations will be available in Cornerstone starting Sept. 1. Mid-Year Evaluations are required for all classified CDHS employees hired prior to Sept. 1.
The Mid-Year Evaluation is an opportunity for the employee to self-reflect, and for the supervisor to give an employee feedback on their performance thus far in the performance cycle. In the Mid-Year Evaluation, supervisors and employees will check in on core competencies, goals, and professional development objectives.
Prior to Sept. 1, log into Cornerstone to make sure your goals and professional development objectives are accurate and progress is updated. For more information on how to review your goals and objectives in advance of the self appraisal, check out this how to guide.
The deadline to complete self-appraisals is Sept. 30. All Mid-Year Evaluations need to be completed and signed by the employee in Cornerstone by Oct. 31.
