Form 10C, Application for Retirement after DROP

LSERS is pleased to announce the release of our new Form 10C, Application for Retirement after DROP which is located on the Forms page on LSERS website.  
 
Here are highlights of significant improvements: 
  • The form has changed names to Application for Retirement After DROP from After-DROP Notification, which is consistent with the other retirement applications.
  • The form will only be used to notify LSERS of the member's impending retirement. 
  • If the member is continuing to work after DROP, complete the online Enrollment. 
  • Employer verification is no longer required.  LSERS will send a copy of the acknowledgment letter to the employer notifying them of the retirement. 
  • Addition of the W-4P, Federal Tax Withholding Certificate which reduces additional paper expenses and streamlines the process.    
We hope you like this new form.  Our goal is to streamline processing, reduce paperwork, and simplify the information provided to LSERS.

Contribution Exception Report Available Online
 

Employers can view their Contribution Exception Report online.  With the end of the fiscal year approaching, this is a good tool to use to help clear up exceptions. The procedures for accessing the report and a chart for resolving exceptions can be found here.

Estimated Employer Statement Available Online
 

Estimated employer statements for fiscal year 2014-15 are available online.  With the end of the fiscal year approaching, this is a good tool to use to reconcile LSERS records with employer internal records.  The employer statement is listed under REPORTS in LSERSWeb and the procedures are located here.

Coming Soon - Return-to-Work Retirees Online Enrollment and Form 15, Salary Reporting

Effective July 1, 2015, employers can begin enrolling return-to-work retirees online through LSERSWeb.  This applies to both retiree groups (earnings limit and certified shortage of full-time bus drivers).  The online enrollment will replace the paper Form 15N.
 
Also, the Form 15 used for monthly reporting of salary earned by those under earning limits will be replaced with online reporting.  Currently, employers already report salaries and contributions through regular monthly reporting for retirees who are working under the provisions of La. R.S. 11:1007 as a full-time bus driver due to a certified shortage. This change for reporting purposes will only affect the earnings limit retirees where their salaries are currently submitted by Form 15 on a monthly basis.

Revised Fact Sheets and Forms

The following Fact Sheet and Forms have been revised and are now available on LSERS Website:
 
Fact Sheet 5, Transfers of Service Credit and Reciprocal Agreements – payment for actuarial fee made payable to LSERS instead of Hall Actuarial Associates.

Form 9T, Application for Transfer of Creditable Service – changes include new letterhead, all instructions moved to 2nd page and added 408(b) and 403(a) plan types.

Form 9M, Application for Purchase of Military Service – purchase of USERRA military service, breakdown of purchase into two increments, actuarial fee may be required and are to be made payable to LSERS.
 
Form 9SB – Application for Purchase of In-State Service – revised instructions, added actuarial fee may be required, added additional types of purchases (LWOP, Accrual upgrade for previous transfers, & involuntary furlough) and ability to have cost broken down into two separate costs.

Form 9RO, Rollover / Trustee-to-Trustee Transfer Acceptance – payment for actuarial fee made payable to LSERS instead of Hall Actuarial Associates
